Bilingual Assessment for Level 1 or 2

Bilingual Assessment Team Referral for Special Education Assessment(s) for School-age Students: ESOL Level 1 or 2 ONLY

Send this completed referral form and the documents noted below via interoffice mail to the Bilingual Assessment Team (BAT) unit at Rocking Horse Road Center. A complete referral packet will avoid unnecessary delays. The case manager should contact the BAT unit three working days after the referral is mailed in order to confirm receipt of the referral by the BAT unit. If a referral for Individualized Education Program (IEP) assessment and a complete packet are not received by the BAT unit in a timely manner (at least 15-business days for educational, speech, or psychological assessments and at least 10-business days for language dominance determination prior to IEP meeting), referrals for special education assessments may be assigned to the school-based team to complete with interpreter(s).

Request for Interpretation Services

Electronic submission of this form is required for MCPS staff requests for interpretation services at meetings or parent conferences. Large-scale event requests must be submitted by the department director. Requests for interpretation services must be submitted at least 10 working days prior to the date of the meeting.

General Education Teacher Report

This form is used to provide a synopsis of student performance.  General education teachers complete the teacher report before IEP and EMT meetings with relevant work samples.  Documents to be used at IEP meetings will be sent home to families 5 business days before the meeting.  Case managers place a copy in the confidential file and upload a copy to OASIS.

Documentation of Interventions

Use this form for both general education and special education to document interventions that go above and beyond best teacher practice.  Documents to be used at IEP meetings will be sent home to families 5 business days before the meeting.  Case managers place a copy in the confidential file and upload a copy to OASIS.

Release of Confidential Information

Use this form to obtain permission to speak to personnel outside of MCPS.  Most often used to speak to outside providers or before completing rating scales.  Case managers place a copy in the confidential file and upload a copy to OASIS as appropriate. 

Classroom Observation

Optional form to use when observing students in a classroom setting.  Documents to be used at IEP meetings will be sent home to families 5 business days before the meeting.  Case managers place a copy in the confidential file and upload a copy to OASIS.

Functional Behavior Assessment (FBA)

The FBA is a systematic process of gathering information to guide the development of an effective and efficient BIP.  Documents to be used at IEP meetings will be sent home to families 5 business days before the meeting.  Case managers place a copy in the confidential file and upload a copy to OASIS.

Behavior Intervention Plan (BIP)

The BIP is a proactive plan designed to address problem behaviors exhibited by a student in the educational setting through the use of positive behavioral interventions, strategies, and supports.  Documents to be used at IEP meetings will be sent home to families 5 business days before the meeting.  Case managers place a copy in the confidential file and upload a copy to OASIS.

Leave Request not Requiring Authorization

To be completed when an employee is requesting annual or personal leave for any number of days/hours, bereavement leave for 5 days or fewer, or all other types of leave for 4 days or fewer. Leave requests not meeting these criteria should be submitted on MCPS Form 430-1. Attach copies of appropriate documentation and submit to your principal/director/supervisor. Approved form must be attached to employee’s timesheet.

Leave Request Requiring Authorization

Please complete form, discuss leave plans with immediate supervisor, obtain signatures, attach proper documentation, and forward to ERSC. You must submit page 2 with appropriate signatures. Keep a copy for your records. Refer to reverse side for detailed instructions. Understanding of leave policies is your responsibility. Going on leave may impact your benefits and costs, sometimes significantly. Make sure you understand the potential impact by reading this form thoroughly and reviewing the Employee Benefit Summary for leave rates. Bereavement leave requests in excess of the contract allowance should be submitted on this form.

Professional Leave Form

This form is used to request and approve professional leave (PRO) for less than 5 days. Appropriate documentation must be attached.  Many trainings require you to bring a signed copy of this form.
